Sierra Pathways’ integrated care model treats co-occurring conditions simultaneously through coordinated clinical services, ensuring that mental health symptoms and substance use are understood in context rather than in isolation. Integrated Care

Sierra Pathways’ Intensive Outpatient Program (IOP) is designed specifically for Transitional Age Youth (TAY) ages 18–25. Unlike traditional adult programs, our TAY-focused IOP addresses the unique developmental needs of young adults by combining evidence-based mental health and substance use treatment with life skills development, peer connection, and experiential learning. Programming includes support for independence, education and vocational goals, and outdoor and adventure-based activities that promote confidence, resilience, and long-term recovery.

Intensive Outpatient Program

Sierra Pathways’ Intensive Outpatient Program (IOP) offers evidence-based substance use and co-occurring mental health treatment for Transitional Age Youth (TAY) ages 18–25. Built specifically for young adults—not adapted from adult models—our IOP combines developmentally responsive care with life skills, education and vocational support, experiential and outdoor activities, and access to Medication-Assisted Treatment (MAT) when clinically appropriate. This integrated approach supports independence, engagement, and long-term recovery.
